Premier PT is a direct access practice which does not require any doctors prescription to be seen.

Since 2003, under the Public Laws of New Jersey, in most cases a referral will not be required to start your Physical Therapy care. Now you can have your injuries evaluated and treated immediately by a NJ licensed Physical Therapist

You may come directly to your Physical Therapist and receive an Evaluation and Treatment for any Musculo-Skeletal injury. Under Direct Access, we have the authorization to treat you for up to 10 visits within 30 days. Upon completion of your Physical Therapy Evaluation, we will determine your Treatment Plan. If further diagnostic tools (e.g., x-ray, MRI, etc.) are needed to ensure proper care, Premier PT will communicate our findings with the physician of your choice. If treatment is required past 10 visits or 30 days, Premier PT will communicate our care to your physician.

Commercial Insurances and Medicare are Welcome

Medicare

We accept assignment and are proud to serve as a Medicare provider. Most medi-gap / secondary plans are also accepted to offset what Medicare does not cover. We comply will all Medicare rules and regulations with regards to your Physical Therapy care.

Private Insurance

Due to the ever changing requirements and limitations of private insurance policies and in order to maintain our commitment to truly providing one to one personalized care, we have opted to be “Out-of-Network” providers for all other private insurance companies. Even though we do not participate in your insurance network, we are fully credentialed by insurance companies to submit claims and receive reimbursement for our services. We believe that you and your physical therapist are the best individuals to determine your care, not an insurance claims adjuster.

As a courtesy to our patients with private insurance, our billing specialist will verify your out of network coverage, and we will prepare any necessary documentation and submit claims for patients electronically the same day of your visit. To be completely transparent, prior to your first visit, our staff will inform you of your Physical Therapy benefits and discuss your financial responsibility.

Health Savings Account / Flexible Spending Account

Our services are covered by health savings accounts and flexible spending accounts. If you have these benefits, they are a convenient and sensible way to manage your healthcare finances.

Motor Vehicle Accident / No Fault

Under the guidelines of NJ PIP, we are eligible to provide physical therapy care to patients who have been involved in a motor vehicle accident. Our staff will work with your claims adjuster to ensure coverage of your treatment.

You can have your musculoskeletal injuries evaluated and treated immediately by a NJ licensed Physical Therapist. Upon completion of your Physical Therapy Evaluation, we will determine your Treatment Plan. If further diagnostic tools (e.g., x-ray, MRI, etc.) are needed to ensure proper care, Premier PT will communicate our findings with the physician of your choice.

Please arrive 15 minutes before your first scheduled appointment in order to complete all necessary paperwork OR easily download our intake paperwork in the PATIENT INFO tab and complete at home and bring to your first visit.

Photo Identification (Valid Driver’s License or Valid Passport), Prescription/Doctor Referral (if available), & Insurance Card

Your first visit will last approximately one hour. It will entail a comprehensive evaluation, which includes an interview and physical examination. Your physical therapist will review your medical history, diagnostic tests when available, and physical therapy prescription from your physician, if available. Treatment will begin on that day and you will be given a Home Exercise Program to follow.

Each patient is treated as an individual. As a result, the length and the frequency of physical therapy sessions vary by patient. Typically, sessions last approximately one hour and occur at least 2 times weekly. This may vary with your physician’s prescription and/or health insurance coverage. We will work with you to make scheduling as easy as possible.

It is best to wear comfortable/athletic clothing that allows easy access to the injured body part(s) and does not restrict movement. For shoulder injuries, a short sleeve shirt or tank top is preferred. For leg injuries, shorts are preferred. (A restroom is available should you need to change.) Please wear appropriate footwear, such as sneakers, that will allow you to perform exercises safely.

There are several factors that affect the cost of treatment. The amount covered by health insurance varies by provider and plan. We will verify your insurance benefits prior to your arrival to ensure that the care provided is affordable. Depending upon your insurance, a co-payment may be required at each visit. We accept cash, checks, or credit card and also accept health savings accounts or flexible spending accounts should you participate.
